To apply for a library card, come to the circulation desk and fill out a registration card.
If you are 18 or older, you will be asked to provide the following:
- your full name with middle initial
- Social Security number or driver’s license number
- date of birth
- mailing address
- phone number
- name and phone number of your employer
- name and phone number of a local reference who does not live with you
If you are under 18, you will need:
- your full name with middle initial
- Social Security number (or driver’s license number, if applicable)
- date of birth
- mailing address
- phone number
- name and phone number of parent’s/legal guardian’s place of employment
- signature of a parent or legal guardian
There is no charge to open a new library card. Replacement cards are available for a $1 fee. |
